After 5 months of being sick-free, all those common ailments come bombarding me at one go.
It feels as if my body is an incubator for all these bacteria and viruses that attack me when my immune system goes down and believe me, they are having the time of their lives making me feel horrible.
So far, in chronological order, I've had viral fever with body aches, abnormal sweating (for someone who doesn't sweat a lot), headache, sore throat, cold, and cough.
Which eventually worsened to the point that I could not swallow water and my pharynx was so inflamed that it was red in color.
By that time, gastric was already joining the party.
I am so sick of being sick.

Responsibility
I'm home again for the weekends but there's still a matter that's niggling my brain.
See now that I'm a "pegawai" I have much more responsibility to take on. Unfortunately responsibility comes in the form of paperwork. And paperwork is bad news.
Now I'm stuck with having to prepare reports on why there were 24 patients who waited more than 30 minutes for their medications at the Outpatient Pharmacy for three days. I had to practically man the front counter alone with another assistant pharmacist and take care of the back at the same time which is not possible for me at this stage. And the specialist from Kuantan, who comes only once every three months, had to choose to come this week when we are really short-handed at the Outpatient Pharmacy. And he has to prescribe more than 7 meds for each of his patient (due to chronic renal disease) and most of these meds have to be recorded into the bincard for each usage. Hence, the waiting time exceeded 30 minutes.
And now I'm the head of Diabetes and Hemodialysis Unit MTAC (MTAC being Medication Adherence Therapy Clinic). And also doing Outpatient Pharmacy when I'm not doing MTAC.
Grr. Being the boss sucks.

Wishes Do Come True
When I made wishes in the past year, I kept asking to go back to "KL".
And my wishes did come true.
I was posted to KL and am now working at HKL.
The irony?
The KL that I got is not Kuala Lumpur but Kuala Lipis.
Next time I'm going to be very accurate and precise with my wishes.

Next Stop: Pahang
After a long and torturous wait, and finally giving up hope of ever getting your posting as a fully registered pharmacist, I finally got my posting out of the blue, when I'm least expecting it.
I got Pahang!
Which was actually my first choice..call me crazy but my only aim was to get out of Terengganu at that time.
It took me some time to absorb it all, being stupid and ungrateful for it in the beginning and finally accepting it in the end because it is still much better than staying in Terengganu or being shipped off to Sabah.
Still, Pahang IS a big state. I could even be posted to the most rural part of Pahang! That's a chance I have to take now..
On a different note, i finally climbed up Bukit Besar for the first and the last time in Terengganu. I felt like Flubber (the green jelly-like thing) the whole way up and especially all the way down. Gah I still need more work to lose more Flubber part of me!

Hit-and-Run
I so did a hit-and-run today.
Nope, no one was injured in this case.
Yup, shame on me.
In my defense, the KKM (Kementerian Kesihatan Malaysia) owned van was only a bit scratched.
You can't really notice it unless it was pointed out to you. OK it was a wee bit obvious. But it is just one more to the multitude scratches on the van.
Whereas my car took the harder hit and it is now a wee bit dented on the back of the left side. And scratched. Ouch.

My Final On-Call
I just finished my final on-call as a provisionally registered pharmacist at HSNZ on 11th September 2010. And I had to communicate with the most arrogant medical officer ever.
Since it was the Raya holidays, most Muslim medical staff were let go of for the holidays and the Chinese ones had to stay to carry out on-call duties. And as usual during the festive season, the Casualty Department became a hectic place due to the many accidents, food poisoning, and gastritis cases courtesy of the festive season. With this hectic crowd, there are bound to be doctors who have to be borrowed from other departments and health clinics during this Raya season and mistakes in prescriptions are unavoidable.
There was this one case that totally irked me. This particular medical officer prescribed many slips with C. Ponstan II/II BD and T. Vit C I/I TDS. For two days in a row. So I decided to call up the doctor to verify the unusual doses during my on-call. Here was how the conversation went.
Me: Good evening Dr. .... I'm Ling from Satellite Pharmacy 7 and I would like to confirm the dose of a few prescription slips that I received from you.
Dr: Yes?
Me: (Proceeded with the problem at hand). Usually we give Ponstan as II/II TDS, do you really want to give it as a BD dose?
Dr: I know that the max dose is TDS, so it is not wrong to give it as a BD dose right since it is still within the max dose? I have always given it to my patients as a BD dose anyway. But since you say it is TDS then it is TDS.
Me: Alright. Then what about the Vit C? May I know what is the indication for I/I TDS? Cause usually I/I OD is adequate as a prophylactic treatment.
Dr: I usually give it I/I TDS as an anti-anxiolytic.
Me: (Reinforcing statement) But Vit C I/I OD is really already adequate enough as a prophylactic treatment.
(It's not like I can tell that doctor that:
Me: Alright.
Dr: (Hangs up immediately)
Rude much?
But other than that, I have had quite a satisfying final on-call with new cases to handle and there were these two particular patients that made my day.
One made me took three puffs of MDI Salbutamol (which is overdose even for normal asthmatic patients and I was not using placebos) to get him to understand the MDI technique but he finally got it in the end. He thanked me and then went off happily with his meds.
Another was a Malay gentleman from Kuantan who had to get cough and cold meds from this hospital and he voiced his concerns that his inhalers weren't working for him anymore even though he took all his meds accordingly (and he was using the best inhalers too). So I took the time to explain to him about his condition and that his inhalers might not work as well due to his untreated cough and cold and his inhalers will work just as well when his condition improves. He thanked me for spending the time to explain his condition and for his supply of meds and went home relieved of one his worries.
It feels good to be appreciated and it is these small things that make you remember why you still pull yourself through with your job even when the conditions are less than appealing to you. And it definitely pays to spend a little more time to make a difference in patients' lives.

CDR Part II
Usually, we do not receive chemo orders on Thursdays because that's just the way it is. And if we do, we will only entertain those that come in before 12pm (on normal days).
However, as I was happily on my way out for lunch at 1pm sharp today (after 1 week of not being able to go out for lunch at the normal lunch time), the phone suddenly rang.
Me: Selamat petang, Farmasi Chemo.
Staff Nurse: Selamat petang, saya SN ..... dari Wad ..... Boleh tak kita indent ubat chemo untuk seorang patient sekarang? Patient baru masuk tengah hari tadi dan doktor baru habis ward round.
Me: Patient kena guna chemo hari ni ke?
SN: Ya. Keadaan patient sesuai untuk chemo..(Proceeded with patient's lab results)
Me: OK..Apakah regime chemo patient ni?
SN: FOLFOX.
Me: (OMG what the tooting hell?!) Bolehkah tunggu sebentar, saya tanya pegawai saya dulu?
Thereafter, the conversation went on.
Conclusion:
I had to prepare the worksheet and labels and take out the necessary items for reconstitution. During my supposed normal lunch time. On a Thursday.
Luckily, I did not need to do the reconstitution. If not...But there's no point having lunch at 2pm cause I get off work at 3.30pm. Crap happens.

Cytotoxic Drug Reconstitution
Yes! I made it through my first ever cytotoxic drug reconstitution process without injuring myself or killing myself with the cytotoxic drug!
Somehow CDR is still kinda boring. It's cause I have to wait for the orders to come until noon before anything can be carried out. And all I can do there is read the the manuals, protocols etc. until noon arrives and the reconstitution work can be carried out.
I can finally get back from work on time but I feel weird cause I'm so not used to this much free time. I still have 1 report and 1 CPD to do and I'm still slacking away...

Office Girl
During the past week in my new rotation at Drug Information Services I have been:
- Making photocopies of documents etc.
- Binding documents.
- Cutting carbon paper into tiny little squares.
- Adding up sums for statistical purposes.
- Preparing and sending out papers to be signed.
- Preparing notices for presentations.
- Becoming a sort of telephone operator and letter receiver.
- Updating the formulary which contains 1467 drugs (had to convert it from pdf to excel, rearrange it in excel then categorize it according to Malaysian Drug Code indication and discipline).
- And so far, I have only answered 3 drug info queries in the past week...

Can't stand fakers
I seriously can't stand people who like to fake it.
Seriously, can you do everyone a favor and get a freaking life?
And about that misunderstanding picture, if that picture is already that old and the topic is so yesterday news, can you not stick your sore butt in and add outdated comments on it?
Seriously, just fcuk off.
And provisionally registered pharmacists in HSNZ are not allowed to dispense at the ACC Pharmacy (outpatient) even though they have been in the hospital for almost half year and this is their second rotation at ACC.
I'm already in my 7th week at ACC and yesterday I was told that I could start labelling already after doing filling for 2 weeks. Like, are you seriously kidding me? I did more dispensing during my first rotation there (also my first month of my hospital placement) compared to after 4 months of training.
We're being paid a pharmacist's salary for doing an assistant pharmacist's job. Twisted.

Payday
Finally, it's payday!
Thanks to the Deepavali holidays, we get to have our pay in advance so now I have 2 months pay!
Having mixed feelings though...I was happy because I'm finally earning my own keep but I did not felt as excited as I should have been when I got my first pay.
Maybe it's due to my long and hectic day at work today. In addition, I have an on-call this Saturday at the Methadone Clinic. Hate on-calls.
Just waiting to go back to KL this Deepavali...

Posting

Got my hospital posting today and guess where I got thrown off to?
TERENGGANU!!!
I have no connections whatsoever to that state, and if you want to trace back my origins I think I would have more in common with China compared to TERENGGANU.
And I will have to report to work within 1st-15th Sept.
There's also no point in appealing to KKM unless you're one of their own.
Really pissed off at the moment. And definitely throughout posting.

Another Unproductive Day
I am such a lazybum. I can't help it. Updating blogs is just not my thing.
Not many things have been happening lately except that the "much-awaited" letter from SPA (note the "enthusiasm"?) has finally arrived last Friday.
This means that we will probably have to start working around end July-early August. And my holidays just started not long ago. No fair.

Just trying to delay my work
I am so freaking tired...I want my sleep back!
I can barely keep my eyes open while I am typing my hospital placement diary (from last week), diaries are such a pain in the ass. I can't even keep up with whatever diary that I have ever owned in my whole 22 years and now I am forced to update my placement diary everyday (which I still don't unless I have free time there). I don't even update my blog often enough!
I am going through Outpatient rotation this week and my god you have to dispense medications and counsel patients in BM there! OMG! My BM is so terrible let alone Chinese language, I chose to be a pharmacist in the wrong country! Guess I'll have to work with what I have. Tomorrow's my turn to dispense and counsel on MDI use. In BM. Keeping my fingers crossed!
Staying in Vista kills. There is nothing here! It's almost like a prison, except that the living conditions are not as bad. Still...people who live here have no life whatsoever, now I know why they can be so studious and score so well during tests. Because all they can do here is study. I am not saying everyone but most people who live here
Just got off on the phone with mom and i was complaining of the boredom here and she told me "Who ask you to stay there?". It's not that I want tooooooooo......I was forced tooooooo....I miss my home...ma, I want to go home!

This is just great
Isnt' it great when your patients can finally be discharged from hospital?
Especially when your cases have to be clerked till they are discharged.
I was so glad just now that two of my patients can be discharged.
But at the final moment, they were forced to remain at the hospital for further observation.
One minute I was so elated and the next it's like someone threw a huge bucket of iced water in my face. Ouch.

Yet another whiny post
It has been so long since I last blogged that I almost forgot how to create a new post.
It was such a busy semester with all the research stuff going on and the pain is agonising. Thank God research is finally over. How did I live through those days of inadequate sleep and doing lab work from 9 in the morning till 9 at night and the one month break that was meant to be a break for me ended up as a non-break because I had to deal with the whole research dissertation and some other things...but anyway all of that is in the past now.
Now I am in my final semester and I have to go for hospital placement in December. When everyone else is going for holidays and haveing their vacations. And I am stuck in a hospital clerking cases. Of course I am not the only one, everyone in B105 has to go through the same thing. Guess I can't complain as much then. But still...
Anyway, I'm still counting down the days till you come back PaoEe. Faster come back...........

Friend?
True friends are hard to come by these days. A friend is defined as a person, who is usually not a member of your family, whom you know well and whom you like a lot. Like it or not, friends influence us in many ways, and some "friends" will try to change you into something you are not and most of the times the new you is not a person you can like or be proud of.
I do not like it when "friends" spread hurtful words and rumours about other friends no matter how true they are. And the worst of these type of "friends" are those that can do this with malicious intent and glee. Utterly disrespectful. It is almost like an addiction to them. They do this because they feel that they are alone and they do not want you to have other friends other than him/herself. These "friends" talk bad about other people to you so that you cannot face them without feeling ashamed or being fake as now you know something that you should not know about them, and eventually you will drift away from other people and you can only turn to these supposed "friends".
Light gossipping is one thing but "friends" who spread venomous gossip about others is an entirely different story altogether and beware of these "friends" as they can declare that they are your "friend" in this moment and backstab you in the next second when you are not looking.
Another type of "friend" that I despise are those that put you or other people down in the face just to make him/herself feel better about themselves. These "friends" are those who know that they are lacking in many ways but they choose to put someone else down or hurt the feelings of others just to prove to themselves that somehow they are superior beings to others instead of working on their weaknesses or trying to look at themselves in a positive light. Hopeless.
Also, there are "friends" who take other people for granted when they themselves do not want others to take them for granted. What hypocrites.
The traits of these "friends" is too long to list in one post. Human nature can be so ugly sometimes.

These "friends" are those who know that they are lacking in many ways but they choose to put someone else down or hurt the feelings of others just to prove to themselves that somehow they are superior beings to others instead of working on their weaknesses or trying to look at themselves in a positive light. Hopeless.
Also, there are "friends" who take other people for granted when they themselves do not want others to take them for granted. What hypocrites.
The traits of these "friends" is too long to list in one post. Human nature can be so ugly sometimes.
